Deciding whether your implants are placed over or under the chest muscle is one of the decisions best made before going under the knife. Nevertheless, in rare cases a patient who opted for one placement option may want to change her implant position through a secondary surgery.

When considering implant placement, you should be aware of the advantages and disadvantages of each placement option.

  • Over-the-muscle implant placement might create more noticeable cleavage and may make rippling of the implants more noticeable.
  • Under-the-muscle placement can sometimes provide for a more natural breast slope, though recovery time in some cases is slightly longer.

When making up your mind on which placement to choose, some aspects of your decision should be made based on your concerns and desires and some should be based on your unique physiology. Women who become unhappy with the result of their breast enhancement surgery may want to consider a revision procedure, particularly if they are experiencing breast sagging, bottoming out, rippling or capsular contracture. Changing the implant placement (or changing out the implants altogether) may help to remedy these concerns.

It is important to consult a skilled plastic surgeon when considering another breast surgery after an initial augmentation. Whether they're looking for a breast implant "redo" or a breast lift in Sacramento, I encourage my patients to ask questions and do a lot of research on their own to ensure that they make the right decision.
