Despite the fantasy of a full head of hair, older patients tend to have more realistic expectations relative to the aging process. Although total donor area and absolute density within this area are obviously important factors when assessing the number of transplanted hairs per area of bald scalp, subtle factors including hair shaft diameter, wave, color, skin/hair color contrast, the emergent angle of the hair, and oiliness and integrity of the hair shaft also contribute to the illusion of 'relative' density when considering the final cosmetic impact of the transplant. The PAI Medical Group utilizes a large session transplant philosophy which encompasses the judicious placement of micro and various-sized mini, follicular unit and standard grafts based on the system of LEVELS 5-12 (ie area of balding scalp to be transplanted) given the constraints of the patients expectations, age, extent of baldness, and the absolute and relative density of the donor hairs. A frontal hairline transition zone consisting of micro, small mini and/or follicular unit grafts is meticulously placed anterior to a zone of larger grafts thus creating a natural full look. Large sessions have relative merits. They minimize the disruptions and postoperative inconvenience associated with multiple surgeries yet still produce cosmetically appealing results. The large sessions can, in certain individuals, minimize the effect of the thinning of the hair shafts that is noted in the early stages of balding (ie miniaturization of telogen effluvium) before hair loss. This is accomplished by transplanting through these areas with a number of mini or follicular unit grafts that is greater than the anticipated number of hairs that may be lost to the effects of miniaturization. Large sessions minimize the concomitant decrease in donor site density associated with the follicular trauma caused by multiple small sessions.
